
LIFE! Taking Action on Diabetes

The LIFE! Taking Action on Diabetes program targets people with risk factors for the development of type 2 diabetes. LIFE! is an evidence based, lifestyle modification program, aimed at preventing diabetes, through interactive, educational group sessions.
Five, two hour group sessions are held every second week, with a sixth session held after eight months. The program empowers individuals to change their lifestyle, promoting healthy eating, engagement in physical activity and achievement of healthy weight. The accredited team includes a dietitian, diabetes educator and nurse. Group support and individual attention ensure participants remain motivated and succeed in lifestyle change.
LIFE! accepts referrals from Doctors for the client aged 40-49 years (attracting MBS items 710, 713, 717), and also clients aged 50 years and over. Dietitians can only refer clients 50 years and over. All clients must score 15 or more on the Australian Type 2 Diabetes Risk Assessment tool, and be excluded from a diagnosis of diabetes.
LIFE! is free to concession and pension cardholders and is available in Shepparton, Numurkah, Nagambie, Cobram, Seymour, Murchison, Rushworth and Tatura. Referral forms available from Goulburn Valley Division of General Practice.
