The Care Journal
The Care Journal, is a major new resource, which has been designed and developed by FamilyCare Carer Support Services. It is a communication and record book of care needs for ageing persons and family carers.
Many persons who are ageing and family carers have said that they are confused about services available to support them and, don’t know how to access services. The aged care support system can be difficult to navigate and information from multiple organisations can be overwhelming. The consumers’ own solution to this has been to keep a folio to help them keep track of all that was happening. FamilyCare have taken those good ideas and designed this journal, through funding from the Hume Region office of the Department of Health and Human Services.
The journal is designed to be a multipurpose type of diary or planner. It can be used;
- as a communication tool for services involved in care, the client and carers to assist in co-ordinating care needs
- as a record of services that are in place
- as a resource to find help in the aged care service system
- as an important aid to keep the client’s GP informed.
