Healthy Ageing/HACC Best Practice
The GVPCP membership decided to focus on local health priority issue which is Healthy Ageing. As a specific population based group, the working group will address the statewide health promotion priorities in reference to older adults. The proportion on older people in the GVPCP catchment is expected to be higher than the Victorian estimates. The Healthy Ageing Working Group is responsible for the implementation of the health ageing strategies outlined in the GVPCP Community Health Plan. The Healthy Ageing Working Group meets bimonthly in Shepparton. The Chairperson for the Healthy Ageing Working Group is Shane Hughan CEO of Valley Sport.
Healthy Ageing Focus for 2008:
The Healthy Ageing group will be looking to amalgamate itself with the HACC Services Best Practice meetings. It was agreed that this would be a more appropriate partnership platform for dealing with the issues surrounding Aged care services. The background to this is that a number of HACC Service providers presented the idea to GVPCP that they needed a new forum to discuss the issues relating to their industry. They were aware of the GVPCP Working group review process and were keen to see some new partnership develop.
After a meeting of the Healthy Ageing group was undertaken it was agreed that the following issues would be standing items at subsequent meetings:
- Socialisation of the elderly (opportunities and issues)
- Potential links between Service providers
- HACC Training Coordination
- Service Gaps and Policy development
- Activity Promotion Development
This would enable the group to look at issues such:
- Service Coordination
- Capacity Building
- Integrated Health Promotions
- Platform and partnership development
- Collate data for demonstrating to funding bodies gaps in service
Initially the group will continue to meet bi monthly, but as discussed, if they can see a positive outcome then they are keen to lift the regularity as they can see this forum having the potential to supersede many other meetings that they attend.
