Chronic Condition Self Management (CCSM)
Area Serviced: Greater Shepparton, Moira & Strathbogie
A partnership between Goulburn Valley Health and Community Health. The CCSM Program provides Health Coaching, Flinders Care Planning and the Better Health Self Management Course.
Eligibility: People who have been diagnosed with a chronic condition(s) and are above the age of 18 years.
Please note carers of those who meet the above criteria are eligible for the BHSMC.
Currently delivered at Goulburn Valley Health, accepts consumers from Moira and Strathbogie. Runs four times a year from 10:00am- 12:30pm. Please contact Goulburn Valley Health for details. General Practitioner referral is required.
